My current working theory for # 3 is something in the Basilica chain of events. There are memory orbs you can intereact with that spawn Seer ghosts - one in the first room and 2 in the last. I wondered if perhaps finding all of them triggered the collection item at the end, but no luck so far. I searched all rooms going through - even sat in all the chairs - but could not find any more.
