The Origins of Madness was initially a Living World achievement category for the The Origins of Madness release. Since the Wrath of the Twisted Marionette bonus event the category is permanently available.

- ^ Energy Probe Dismantler: The Energy Probe located just south of Sanctum Harbor Waypoint in Memory of Old Lion's Arch cannot be interacted with, making it impossible to use for this achievement.
- ^ The Secret Lair: Also rewarded a Bag of Power Core Pieces during the original release. This reward was removed for the category's return in the July 13th, 2021 update.
